Collection Statement:

The practice will collect personal information as a provision of clinical services to a patient of the practice. Information that will be collected will include at a minimum the following: name/s, address, DOB, phone number, Medicare number and next of kin/ emergency contact details. Other medical information including medical history, allergies, immunisations, medications, social history, family history and risk factors will also be collected to ensure we are providing our patients with the highest standard of patient care.

Right to Access Information

In line with Australian Privacy Principle 12 (APP 12) patients have a right to access their medical records.  These records can be requested by another medical practice or for the individual.  A signed request is required prior to release of information.

In some cases, requests for records may be denied.  In these instances, the patient will be notified in writing outlining the reasons for this.

All requests for records will be actioned within 30 days.
